Beyond the Darkness: How Light Therapy is Restoring Autonomy in Neurodivergent Patients I’m excited to share a groundbreaking translational case report recently published in Swiss Federation of Clinical Neuro-Societies (MDPI) regarding Down Syndrome Regression Disorder (DSRD). Great work Gabriela Guimaraes, MD, MS and team! DSRD is a severe condition where individuals experience an abrupt loss of speech, motor skills, and functional autonomy. This research highlights a successful multimodal approach to reversing these symptoms by targeting the gut-brain axis. The Case Study: An 11-year-old girl who had lost speech, motor coordination, and continence was found to have a chronic Giardia infection, significant gut inflammation, and microbial dysbiosis. The Multimodal Intervention (90 Days): The team implemented a three-pronged strategy to restore neuroimmune homeostasis: Antiparasitic Treatment: A single dose to eradicate the primary infectious trigger. Metabolic Support: Daily folinic acid to optimize the neurochemical environment and support neurotransmitter synthesis. Photobiomodulation (PBM): Daily near-infrared light (1064 nm) applied both transcranially (to reduce neuroinflammation) and transabdominally (to modulate the gut microbiome and reduce mucosal inflammation). The Results: After 90 days, the results were transformative: Biological Recovery: Gut inflammation markers (fecal calprotectin and secretory IgA) were completely normalized, and beneficial bacteria (Bifidobacterium, Lactobacillus) were restored. Behavioral Breakthroughs: There was a 50% reduction in aberrant behaviors (ABC score) and a significant decrease in autism-like symptoms. Functional Gains: The patient regained continence, resumed climbing stairs independently, and began using spontaneous speech again. Why This Matters: Traditional treatments for DSRD often involve high-dose steroids or antipsychotics with significant side effects. This case provides a "proof-of-concept" for Photobiomodulation as a safe, non-invasive, and low-risk adjunct therapy that can "lower the physiological noise" of inflammation, allowing the brain to regain the capacity for learning and recovery. New research from UC Irvine’s ICAL-- Piomelli, Lynch, and Gall laboratories—reveals a surprising role for microglia in supporting endocannabinoid-dependent synaptic plasticity and episodic memory. In this Journal of Neuroscience study, Chavez et al. (2025) show that depleting microglia in adult mice using the CSF1R antagonist PLX5622 selectively disrupted a singular, endocannabinoid-mediated form of long-term potentiation (LTP) at the lateral perforant path–dentate gyrus (LPP→DG) synapse. Despite intact basal transmission and other forms of LTP, microglia depletion caused: 🔹 Loss of LPP-LTP, 🔹 Elevated levels of the endocannabinoid 2-arachidonoylglycerol (2-AG) in the dentate gyrus, and 🔹 Impaired encoding of episodic “what–where–when” memory elements. Remarkably, enhancing 2-AG signaling with the MAGL inhibitor JZL184 rescued both LPP-LTP and episodic memory deficits. Standards move forward when people do. In San Diego, a community came together to build what scientific publishing needs next. openRxiv and Continuous Science Foundation, funded by The Navigation Fund, convened tool builders, researchers, publishers, and infrastructure partners who share a common goal: to create open standards that make research publishing interactive, interoperable, machine-readable, and push towards modular publishing. This was not a meeting. It was a maker space alive with collaboration. A shared keyboard floated through the room as people vibed on code, and the group tried on bold approaches. Together, we ran real content through emerging early prototypes of a federated standard that supports continuous, modular and open publishing. Working side by side built alignment, shifted perceptions, built trust, surfaced new possibilities, and moved the group toward shared clarity. The aim was to resolve critical technical gaps and demonstrate live interoperability, laying the infrastructure for modular publishing and all of the vibrant applications that are built on top. The heart of this gathering was trust. Trust created the conditions to push thinking, try new approaches, and strengthen interoperability pathways across the ecosystem. Coming together: - Prototypes took shape quickly. - Federated reference architecture became more concrete. - Strengthened commitment to modular and open standards. - Assumptions were replaced with shared clarity. Shared infrastructure like standards enables change, new behaviours, and shared capabilities.
